CORI FORM REQUIREMENTS

FORMS WILL BE RETURNED IF THEY DO NOT MEET THESE STANDARDS!!

In the interest of helping everyone to get their forms processed promptly and without any returns for corrections, please read this list, review the example, and then call the Office for Child Protection (508) 674-4681 if you have any questions at all.  Thank you for your kind attention to these details, many of which are requirements of the Commonwealth of Massachusetts.

GENERAL CORI FORM REQUIREMENTS

  • Must be completed on the correct form, issued 2010;

  • Must be on the correct color of paper(Blue for 2012);

  • Must be completed in ink only, no pencil allowed;

  • NO white-out allowed(please just cross out incorrect info with one line and have applicant initial it, as with any other legal document);

  • Must have complete documentation of ID, including state of issue for all forms of state ID;

  • Must have complete verification of ID, including authorized trainer’s initials following the documentation of the ID on the proper line

  • Must have applicant signature in ink;

  • Must have photo ID attached (front only - we do not need the back of the ID);

  • Must have RI Disclaimer form attached for all RI driver’s licenses and they must be notarized — CSS can no longer do that for you;

  • Must have Code of Conduct on the back, filled out completely, unless it is a subcontractor form.  Those do NOT need the Code of Conduct; and

  • MUST be an original form.  NO copies will be accepted!

VOLUNTEER FORM REQUIREMENTS

  • Must be on Blue paper through December 31, 2012. 

  • Must have current Volunteer Questionnaire attached when the volunteer is submitting CORI for the first time only.  After that, the Questionnaire is not needed.

  • Please remember the final window for sending in the previous year’s CORI forms.  Per section 6, page 1 of the OCP Manual:
  • “Please remember that any CORI that is done within the last two weeks of the calendar year (December 16 – December 31) will be accepted on that year’s color of paper, provided it has been postmarked no later than January 7 of the following year. Any CORI that was completed prior to December 16 and/or that is postmarked after January 7 will not be accepted. It will be returned and the new color form will need to be completed.”  THERE WILL NO EXCEPTIONS!!

GENERAL SUBMISSION REQUIREMENTS

  • Use the proper training sign-in sheet and make sure it is completed correctly, including the trainer’s initials in the far left column labeled “T.I.”  You no longer need to include post-test scores unless you wish to.

  • All sets of forms (either renewals or groups of training sessions) must be in alphabetical order;

  • Forms for each individual must be stapled together as a set as follows:
    1. CORI form
    2. Copy of photo ID
    3. RI Disclaimer (if needed)
    4. Volunteer Questionnaire (if needed - first submission only)

    PLEASE do not staple the ID over the CORI, copy the back of the ID, or send the information for an individual in parts which we must sort and staple.  That is a recipe for disaster when we are bombarded with hundreds of forms in a day. We also do not need a copy of the back of the ID, nor do we need two copies of the ID.

  • The only item that can be paper clipped is the sign-in sheet that can be attached to a group of CORI forms from the corresponding training session.
